On June 27, 2025, HLN released a new version of the Immunization Calculation Engine (ICE) software, bringing important updates and enhancements to ICE vaccine evaluation and forecasting. This latest release, v2.47.1, delivers improved logic, enhancements, and alignment with current immunization guidelines across several vaccine groups: Hepatitis A, Hib, MMR, Orthopoxvirus, Pneumococcal, Polio, and RSV.
ICE is a state-of-the-art open source software system that provides clinical decision support for immunizations for use in Immunization Information Systems (IIS), Electronic Health Record (EHR), and Personal Health Record (PHR) Systems.
What’s New in ICE 2.47.1?
- Hepatitis A:
- Modified series selection rules: The 3-dose series is now limited to when CVX 104 (Twinrix) is administered to an adult patient as dose 1 and/or dose 2. Otherwise, ICE will evaluate and forecast in the 2-dose series, where a 6-month interval is required between doses.
- Added absolute minimum ages and absolute maximum ages to vaccines; the addition of the absolute maximum age will invalidate pediatric formulations of the vaccines when given to patients 19 years and older, in line with recommendations to repeat the dose.
- Modified logic for the 2-dose series and the 3-dose series.
- Hib:
- Improved booster dose handling for CVX 50 in the Hib OMP series.
- MMR:
- Updated the evaluation of MMR at 6 – 11 months from Invalid to Accepted, given MMR may be given at this age for high-risk patients.
- Updated the dose 1 and dose 2 absolute minimum age to months/years instead of days.
- Orthopoxvirus:
- Updated the CVX 75 name to “Vaccinia (smallpox, mpox), live” for clarity.
- Pneumococcal:
- Updated adult logic to align with the latest guidelines, including lowering the recommended age for the first adult vaccination from 65 years to 50 years.
- Added support for CVX 327 (PCV21) and CVX 177 (PCV10).
- Lowered PCV20 allowable age for the childhood series.
- Improved handling of duplicate pneumococcal shots recorded on the same day.
- For the zip file distribution only: added property setting in ice.properties, disable_output_earliest_and_overdue_dates_for_pneumococcal_adult_series. If set to “Y”, ICE will not output the earliest and overdue dates for the Adult Pneumococcal Series. By default, this setting is disabled.
- Polio:
- Updated the CVX 02 name to “OPV, trivalent” and updated the CVX 89 name to “Polio, unspecified formulation (NOS)” for clarity (no logic changes).
- RSV:
- Updated recommendation for patients age 8 months – < 20 months old who completed their infant series from Conditional / HIGH_RISK to Conditional / COMPLETE_HIGH_RISK; this supports identification of children in this age range who completed the RSV infant series as compared to those who did not.
- General Rule: Live Vaccine Interval:
- Update to ensure enforcement of a 28-day absolute minimum interval between an MMRV and another live vaccine.
